I don't see any element of that video that couldn't reasonably be done in-engine. Does it really look dramatically better than Unreal 3.0? Check out Gears of War 2...
Plus this is Carmack / ID Tech 5 so why the hell not??

me neither. looks absolutely stunning. and you can tell it's in-engine because there are several artifacts such as the shadow popping in on the water tower at 0:56.

It'll of course come down to the gameplay in the end. That's impossible to judge from this - but to me, Id usually nails the character movement and have some weapons that are fun to use which is more than I can say about most other shooters. Hopefully this time the character control will be used in interesting encounters instead of closet monsters.

My only fear is that will it offer anything particularly new to the genre? Although the driving sections look like motorstorm with guns, which would be cool.

Are you kidding. It's definitely a mix of lots of other post-apocalyptic worlds but it blends into something unique. The voice acting and character movement are stunning.
As for the driving I think id is stepping into the future. With a fast, well-armed, and agile dune buggy Rage can top games like Halo that have bad mechanics. And it gives a break in FPS. Really I'd rather have 3rd person view in the console version to increase peripheral vision since thumbsticks can't turn as fast.
