If it's an RTS, it probably means the player pays for better units or new races. If it's pastrami, it could mean extra spices, or a longer smoking period.

Probably an MMO that follows the Korean/Asian payment model. The game is free to play but the nice looking loot and some other items are only available through grind or payment. Not buying anything doesn't stop you completing or enjoying the game but if you spend lots of time in the world then there's some enhancements you can buy if you want.

I think that's how Guild Wars and their ilk work and it'll become a more popular payment method in time.
