Elana, the Squalid Queen

When you enter the boss's lair, run towards Elana as fast as you can and just throw out all the damage you've got. It takes her a little while to get started in combat, so anything you can do to hurt her first will give you a very helpful advantage. During the fight she'll summon skeletons to aid her, launch a variety of fire-based attacks at you, and even jab you with a spear if you get close enough.

As soon as those skeletons show up, get away from them quickly so you avoid the ambush. Unless they're focusing on you, you can more or less ignore them altogether now, as you'll likely whack them a little bit while you're hitting Elana anyway. Try to keep moving when she uses her spells, and keep to the rear while attacking if at all possible.

When she vanishes, get ready for a lot of spell damage when she turns up again. The good news is that she's not too quick when it comes to charging up spells, so get a few good whacks in and then get out of the way.

Once you've killed Elana, the massive door behind her will open up. Go through it, light the bonfire on the other side, and then get ready for another boss fight on the other side of that misty door.
